NPHF/Procter & Gamble Endowed Scholarship in Community Service

The Application deadline is November 30, 2010.

The NPHF/Procter & Gamble Endowed Scholarship in Community Service is awarded to one (1) nurse practitioner graduate student who has made a significant positive contribution in school, in the community, or in patient care while in the pursuit of an advanced degree. One scholarship will be awarded. Amount of award is $1,000.

(Endowed scholarship through an educational grant from Procter & Gamble.)

Application Instructions:

1. The forms are in a fillable PDF format. Please fill them out online.
2. Print the application forms in their entirety.
3. Gather the rest of the information you need to submit.
4. Order the forms and information according to the checklist.
5. Make 3 copies.
6. Mail the original and the 3 copies to:

   Bellevue, WA 98005

NOTE: There is no online submission process, you MUST mail the application.
