NPHF/Astellas Heart Health Through the Ages Award Program

The Application deadline is December 15, 2010.

Three (3) awards will be given to practicing nurse practitioners and graduate nurse practitioner students whose service, research, or educational projects are related to promoting heart health. There will be one (1) award each in pediatrics, adult medicine, and geriatrics. Each recipient will receive an initial award of $3,000. Upon completion of the project, an additional $1,000 will be awarded to each recipient to support project dissemination (e.g. travel expenses for presenting at a regional/national meeting, editing a paper for publication; preparing a poster, etc.). The project must be completed within 2 years. Three (3) awards. Total amount of each award is $4,000.

Application Instructions:

  1. The forms are in a fillable PDF format. Please fill them out online.
  2. Print the application forms in their entirety.
  3. Gather the rest of the information you need to submit.
  4. Order the forms and information according to the checklist.
  5. Make 3 copies.
  6. Mail the original and the 3 copies to:
