NPHF/Astellas Heart Health Through the Ages Award Program

The Application deadline is November 1, 2012.

Provides six $3500 awards to nurse practitioner graduate students or practicing nurse practitioners with a research or educational project in the area of promoting heart health in the United States. (Project development and implementation $2500; after project completion, $1000 for dissemination of outcomes.)

Requirements

  1. Grant recipients will be required to submit a one page status report at the end of the first year and a final report upon project completion with a copy of all products (publications, DVDs, slides, etc.) that are a result of this project.
  2. Project must be completed within two (2) years.

Eligibility: Nurse Practitioner Graduate Student or Practicing Nurse Practitioner

(Supported through a charitable donation from Astellas Pharma Global Development Inc.)

Application Instructions:

  1. The forms are in a fillable PDF format. Please fill them out online.
  2. Print the application forms in their entirety.
  3. Gather the rest of the information you need to submit.
  4. Order the forms and information according to the checklist.
  5. Make 3 copies.
  6. Mail the original and the 3 copies to:
